    What to do first

    • Contact the practice directly — a delisting is a business change, not a closure.
    • Request a complete copy of your records, including imaging.
    • Ask in writing how paid, unfinished treatment will be completed.
    • If the practice is unreachable or refuses, file with the state dental board.
