Consumer Health Data Notice
This notice is required by Nevada SB 370 and the Washington My Health My Data Act. It is separate from, and in addition to, our Privacy Policy.

We are not a HIPAA covered entity
1-800-VENEERS™ is a consumer education and provider-matching platform. It is not a dental practice, not a healthcare provider, and not a HIPAA covered entity or business associate. Using this site does not create a doctor-patient relationship, and nothing here is diagnosis or treatment advice. Information you share with a licensed provider after we introduce you is held by that provider under their own HIPAA obligations, not ours.
What consumer health data we collect
- Smile concerns and treatment interest you select in the SmileQuiz™
- Your treatment timeline and budget range
- Your self-reported credit score band (financial, used for financing eligibility)
- The recommendation category our scoring produces (whitening, veneers, All-on-X⁶)
- Contact details you provide: name, email, phone
Once your recommendation is computed we store the derived result — not the raw text of each individual clinical answer. We do not collect diagnoses, medications, insurance claims, imaging, or clinical records.
Medical clearance answers
If you book an appointment, you may be asked screening questions (for example about cancer history, chemotherapy or radiation, or organ transplant). Those answers are scored on our servers for safety routing only. They are never sent to any AI model, never used for advertising, and never shared with an ad platform or analytics vendor.

We do not sell consumer health data, and we do not send your quiz answers, chat messages, or screening answers to advertising or analytics pixels. Advertising measurement receives only a generic conversion event with no answer content.
AI processing
Some features use AI models to answer questions. Any model that can receive your text operates under zero-retention, no-training terms — your content is not stored by the vendor beyond the request and is not used to train models. Models without those terms are blocked in code from receiving health-related text.
Who it is shared with
- Licensed providers you ask to be matched with
- Our email/SMS delivery and CRM processors, under contract, for your communications
- Our hosting and database processor
- Law enforcement only where legally compelled
Your rights
You may withdraw consent, request a copy of your consumer health data, request its deletion (including deletion by our processors), and appeal a denial. Email privacy@1800veneers.com with the subject line “Consumer Health Data Request.” We respond within 45 days. If we deny your request you may appeal by replying to our response, and you may contact the Nevada Attorney General or the Washington Attorney General.
Retention
Derived recommendations and contact records are retained while your inquiry is active and for up to 24 months afterward, then deleted or de-identified. Deletion requests are honored sooner on request.
